Opioid Management of Chronic Pain

Abstract

|
The efficacy of opioids as analgesic agents in conditions of acute pain is well established. However, its effect in chronic pain has not been well described. In this review, classification of opioids in view of its biochemical properties, pharmacokinetics, and the mechanisms of action are presented. As problems such as tolerance to medication, physical and psychological dependence, and acute or chronic toxicity may occur in chronic pain patients receiving opioid prescriptions, a thorough understanding of these matters are crucial for the prescribing physician. The physician should also be familiar with the legal problems associated with opioid medication, and a team approach including a psychologist, anesthesiologist, physical therapist, pastor, nurse, dietician etc., is more effective for the management of pain. Patients need to be informed of the adverse effects of opioid medication and preventive measures including diet control. A greater understanding of opioid medications may allow the physician to provide more effective measures in overcoming the devastating effects of chronic pain.
